Privacy Policy

This Privacy Policy sets out the manner in which Alissa Steinberg. (“Alissa Steinberg,” “we”, “our”, “us” or “Alissa Steinberg, RD”) collects, uses, discloses and otherwise manages the personal information we collect through our website located at www.healthyu.ca (“Website”).

 

Our Privacy Commitment to You

We are committed to protecting your privacy and ensuring the confidentiality of your personal health information. All your personal health information is private, confidential and secure.

 

Collection and Use of Personal Information

“Personal information” means information about an identifiable individual, including your name, address, email address, signature, provision of health care, and other information relating to you. We collect, use and disclose personal information in order to serve our clients. You can withhold or withdraw your consent to the collection, use or disclosure of your personal health information by contacting us. For our clients, the primary purpose for collecting personal health information is to provide nutritional counselling service.

 

We collect, use and disclose personal health information for the following purposes:

  • To provide nutrition counselling, care and assistance based on your needs

  • Ensure that all service providers have the most up-to-date and complete record of your health history

  • To conduct quality improvement To obtain payment for services provided

  • To comply with our regulatory obligations to The College of Dietitians of Ontario

  • To teach students and to provide continuing education to our staff

  • To advise clients about special events, opportunities and nutrition information through monthly newsletters

  • For general administration and management of our programs and services

  • For other purposes permitted by law

 

The types of personal health information we collect may include your:

  • Name

  • Address and other contact information

  • Occupation/profession

  • Credit card or other payment information

  • Age

  • Health history of individual

  • Family health history

  • Health measurements, samples or examination results

  • Health conditions, assessment results or diagnoses

  • Health services provided to or received by the individual

  • Prognosis or other opinions formed during assessment and treatment

  • Compliance with assessment and treatment

 

Access to Health Records: You have the right to seek access to your health records that we keep and to ask us to correct a record if you believe it is inaccurate or incomplete.

 

Sharing of Personal Information: Under no circumstances, will we disclose, trade, advise or otherwise transfer your personal information, without receiving your consent first, except as set out herein.

 

Retention and Destruction of Personal Information: We keep our client files for at least 10 years from the date of the last client interaction or from the date the client turns 18. Clients under 18 years of age should have a parent/guardian to witness relevant information from counselling sessions and/or nutrition plans to offer a collaborative and transparent approach. We destroy paper files containing personal health information by cross-cut shredding. We destroy electronic information by deleting it in a manner that it cannot be restored.

Payment Policy

Registered Dietitian services are often covered by insurance companies, however Healthy U does not offer direct billing to your insurer. In order to get coverage, you must submit your paid receipt to your insurance provider. Payments are processed by our Financial Team, and can be contacted by email at info@healthyu.ca for any questions or concerns.

 

Healthy U accepts both credit card and e-Transfer payment methods:

  1. Credit card option: Your Healthy U Owl Practice account is a secure platform where you can enter and store your credit card information, and payments are processed via Stripe. Our Financial Team will process your payment after your visit, and send a receipt of payment confirmation to your email registered with your account.

  2. e-Transfer option: All e-Transfer payments should be sent to alissa@healthyu.ca within 24 hours of your scheduled visit date and time. Our Financial Team will send a reminder email 1 week after the invoice was sent for outstanding payments. A final reminder will be sent 2 weeks after the invoice. After the final reminder, all future sessions will be put on hold until the late payment is received. After your payment has been received, our Financial Team will send a receipt of payment confirmation to your email registered with your account.

 

Marketing and Promotional Communications: We offer you the ability to sign-up to receive marketing and promotional communications through our Sites. If you sign-up, we may collect certain personal information (such as first and last name, and email address) which we use to send you updates on upcoming events and programs. You can opt-out of this function by following the opt-out or “unsubscribe” instructions.

 

Social Media: We may offer you the opportunity to engage with our content on or through third-party social networking websites, plug-ins and applications, including Instagram and TikTok. When you engage with us or our content on or through third-party social networking websites, plug-ins and applications, you may allow us to have access to certain information associated with your social media account (e.g., name, username, email address, profile picture, gender). We may use this information to personalize your experience on the Sites and to provide you with information that you may request.

 

Customer Service: When you contact us with a question, concern or complaint, you may be asked for information that identifies you (such as your name, address and a phone number) along with additional information we need to help us promptly assist you. We may also retain this information to assist you in the future and to improve our customer service, Sites, product and service offerings, and events and promotions.

 

Links To Other Websites: Our website contains links that lead to outside websites that are recommended for clients to look further into. If you click on these links, Healthy U is not held responsible for your data and privacy protection. Visiting those websites is not governed by this privacy policy agreement. Make sure to read the privacy policy documentation of the website you go to from our website.

 

Duration of terms of service: The Terms and Policies agreement is provided during the full duration of any Healthy U services, however you may choose to terminate the willingness at any point.
